Forest Hills Vision Associates, Inc. provides this website to help you learn about our practice, explore eye care and eyewear, and find ways to request an appointment. This notice explains information used by the website. It does not replace the practice’s separate Notice of Privacy Practices for medical information.
Reading preferences stored on your device
When you choose a text size, the website saves that setting in your browser so it can use your preferred size on later visits. It also remembers whether the optional reading-size invitation has appeared. These settings use browser local storage rather than advertising cookies. The website does not send these preferences to the practice.
You can change your text size at any time using the controls at the top of the page. Clearing this website’s data in your browser removes its saved reading preferences and allows the invitation to appear again. Your browser may also clear this information automatically. If storage is unavailable, the controls still work during your visit, but your preferences may not persist.
Technical information needed to provide the website
When you visit a website, your browser sends technical information to its hosting and delivery services. This may include your IP address, requested page, request time, browser information, and referring page. Our hosting services process requests to deliver pages, maintain availability, and protect the service.
The website is hosted through Sites, using Cloudflare delivery infrastructure. Technical request information may be processed by those service providers to operate, secure, and troubleshoot the service. The website application does not maintain a separate visitor database. Appointment requests and other external services
The appointment links take you to Weave, a separate service used for appointment requests. Information entered there is handled through that service and the practice’s appointment process; it is not entered into a form hosted on this website. Review the notices provided by Weave before submitting information. You may also request help by calling (718) 261-6000.
Links to Google Maps, Google reviews, and other websites take you to those services. They may receive information from your browser when you visit and have their own privacy practices. Maps and appointment forms open through external links. Our Practice also offers a YouTube video that loads only when you choose to watch. Once loaded, YouTube receives information from your browser and handles it under its own privacy practices.
Public Google reviews
If Google review content is displayed here, it comes from reviews published publicly on Google and can include the reviewer’s public display name, rating, review text, and review date. The website retrieves this content through a server connection to Google and uses a temporary cache; it does not send your reading-size choices or appointment information as part of that retrieval. Cached review data expires within six hours.
To edit or delete a review you posted, use your Google account. Changes may take time to appear on Google and then on this website. Contact the practice if you have a concern about a review displayed here. Avoid including private medical information in a public review.
Forms, tracking, and advertising
The current website does not offer a general contact form, patient-upload form, payment form, newsletter subscription, or patient account. The application does not include advertising pixels, session-replay tools, or third-party audience analytics, and does not send website browsing activity to advertising platforms. If these features change, we will update this notice to reflect the new practices.
Information about children’s eye care is intended to help parents and caregivers. The website does not provide a form for children to submit personal information.
